Who is stating that he appeared to be 16? Why are you sure he has a "high metabolism"? He turned the heat off so it would not mask any potential noises indicative of someone sneaking upon him or the rattle of dishes as a husband broke free.
A theory including an endocrine disorder is not news. It has been discussed time and again. While it is not entirely without merit, the evidence in support of it is sparse.

He attacked with every advantage; vulnerable victim, surprise, gun and knife, threats, physical violence, the had the women tie the men, put dishes on their backs, took the women out of the room, controlled the men with the leverage of threats to harm the woman in the other room if the man failed to comply, and still went back and retied the men several times. Why? He knew he couldn't handle a man if it came down to an out and out fight. I'd venture that if he heard those dishes rattle he would have been out of there instantly. He wasn't a man. He wasn't a tough guy. He attacked little girls home alone.
By the way, thank you for the lesson in pop psychology. The lesson in genetics was illuminating, as well.

He may NOT have been 16 at the start, but older with a hormone imbalance which caused the high voice/build and the ability to appear younger. It would explain the high metabolism, turning heat down and things of that nature also.

While it may engender popular feelings to describe them as such it doesn't help when working with the evidence they left behind to think of them that way. The people who try to figure out what makes them tick have described the lack of the empathy gene and also them having the fearlessness gene in combination. It's an over simplification of genetics, but the combo of both traits and the right upbringing or traumatic event helps to create one. Most people with one or both genetic traits never kill people. Many go to professions that are either risk filled like a firefighter or fighter pilot or towards jobs which give them power over others (politics,business,military etc). Not all of the people who gravitate towards these professions and a few others I didnt name are broken in the manner described above but they do have a higher proportion of people than other professions (not anything close to a majority but a very measurable difference). You have to understand what makes them tick in a general sense to help figure out their motivations at the time and in the future. Prediction of future actions or stations in life can help track down the person."
