There was a renowned orthopedic surgeon (just like Dr. Offerman...) in Chicago named Lyman Smith, which is somewhat of a coincidence by itself, but I don't mean him by this thread. I instead have a raw theory concerning California doctors and a possible mix-up which ended up killing (the wrong) Lyman Smith.
Okay, here it goes. ONS was a vietnam veteran, a hospital corpsman (possibly specializing in orthopedy). After this he continued assisting in the medical field, perhaps at Kaiser Permanente*. At some point he had come into contact with Dr. Offerman - and for a reason or another ONS chose to kill him and Manning. I think that at least during his ONS phase, his choice of victims was somehow related to their contact with medical services.
* I would really need to know where the Kaiser Permanente locations were during his crimes, but as of today there is many times only ~10 minute drive from the local KP center to the crime scene. I did a picture (in Sacramento I used the map someone had made previously marking EAR attacks, to which I only added the location of Kaiser Permanente E.R.):

But like I said; I have no idea if the locations today match those of ~40 years ago. But this is just on a side note.
Lyman Smith would relate to this how?
There is a doctor named X. Smith
Orthopaedic Surgeon
Dr. Smith is a board certified orthopaedic surgeon. He is a general partner of Orthopaedic Specialty Institute of Orange County.
...associate clinical professor at the University of California, Irvine.
Following completion of orthopaedic specialization, Dr. Smith served in the United States Army, spending one year in Vietnam..."
A vietnam veteran who is also an orthopedic surgeon. If ONS was a hospital corpsman with a special link to orthopedy, this could explain some previous knowledge about Dr. Offerman, and an intention to kill Dr. X. Smith. But there was a mix-up.
He searched his victims through some directory at the medical center or a hospital he worked at. And he thought that he found the right Dr. X. Smith, when he found this:
"Dr. X Lyman Smith (X here represents the name of that intended doctor)
Kaiser Permanente Medical Center
University of California Davis
Kaiser Permanente Medical Center
University of California San Diego School of Medicine
(pediatrician)"
(...Maybe it was a Kaiser Permanentes list of personnel?)
So set of events summarized:
ONS wants to kill X Smith (remember, also an orthopedic surgeon like Offerman)
ONS searches some directory or register of medical personnel, and finds this Dr. X Lyman Smith. He thinks that he has found the right guy, and Lyman must only be his middle name or something.
Then he takes some other directory, where he can search for Dr. X Lyman Smith's address and finds Lyman Smith. Maybe he has some concentration problems and disorganized thoughts because he is desperate to release his anger by murder, so he hastily ends up to a conclusion that this is the right guy; living at High Point Drive, Ventura County.
So it would be a double error...
